Office Hours and Appointments

Our office hours are listed below. Please call our office at 724.934.3844 to schedule an appointment. Our staff is available to answer any questions you may have.

Medical Records

A complete medical history must be recorded for each patient. You will be asked to complete a questionnaire prior to your consultation. Please make sure all relevant information is available including a list of your medications including dosages and time taken. Please complete our registration form online and submit electronically to our office.  You may also print a registration form to complete by hand and bring to your appointment. X-Rays

A full jaw (panoramic) x-ray is usually required for oral surgery. If your dentist or orthodontist has taken one within the last six months, you may save yourself this additional fee by picking up that film from the dentist’s or orthodontist’s office before your appointment. Our office has a Cone Beam CT scanner, and this specialized 3-Dimensional image may be obtained for dental implant and other specialized procedures and evaluation of jaw pathology.

Emergencies

Our office is on call 24 hours a day, 7 days a week. After hour calls are forwarded to Dr. Bamonte or an on-call physician. If you have an emergency such as severe pain, swelling or fever, please notify our office at 724.934.3844.

Fees and Payments

Our administrative staff will provide any necessary information regarding fees, payments, and processing of insurance. We request that all financial arrangements be made prior to service. A variety of payment and financing options are offered for your convenience. Online verification of your insurance coverage will be performed for most procedures. Written pre-authorization may also be obtained. Additionally, we will submit your insurance claim following treatment as a service to you. Some companies pay fixed allowances for certain procedures and others pay a percentage of the charge. It is your responsibility to pay any deductible amount, co-insurance, or any other balance not paid by your insurance company. We participate with various insurance plans including but not limited to Highmark Blue Cross and Blue Shield, United Concordia, UPMC Health Plan, Delta Dental, Aetna, Met Life, and United Healthcare.  All plans can be different.  Many oral and maxillofacial surgery procedures are covered under your medical insurance.
